The name sounds frightening, and the reality is usually far more ordinary. Here is what degenerative disc disease actually means, why the label overstates it, and how we approach it at Shift.
What It Is
The name is scarier than the thing.
Despite what it is called, degenerative disc disease is not really a disease, and for most people it is not something that keeps relentlessly getting worse. It is the gradual, normal change that spinal discs go through over the years, a little like grey hair or wrinkles, but for your spine. The discs lose some height and water content and can become stiffer over time.
Some people feel these changes, many do not. They show up on scans all the time in people with no back pain at all. So being told you have degenerative disc disease is not a diagnosis of something rare or dangerous. It is a description of wear, and what matters is what your symptoms are actually doing.

A scan report with the word degenerative on it can be unsettling. These three points put it in perspective.
Not really a disease
The label describes normal wear, not an illness that spreads or attacks your spine. Many clinicians think the name is unhelpfully alarming.
Nearly everyone has some
Disc changes are extremely common with age. By middle age, most spines show some of it, whether or not there is any pain.
Findings do not equal pain
How much shows on a scan often does not match how much pain someone has. That is why we treat you, not your image.
What It Feels Like
When it does cause symptoms.
When degeneration does produce symptoms, they are often a low, nagging ache or stiffness in the back or neck, sometimes with flares that come and go. It can feel worse after long periods in one position, such as sitting or standing, and it often eases once you get moving.
If a worn disc contributes to narrowing or irritates a nearby nerve, it can also refer symptoms into a limb, which is where it can overlap with problems like sciatica or spinal stenosis. Just as often, though, the changes sit quietly in the background and cause nothing at all.
Knowing The Difference
When to seek urgent care.
Wear-related back pain is not an emergency. A small number of signs mean you should seek urgent medical care straight away rather than wait for an appointment.
Seek emergency care now if you have
- Loss of control of your bladder or bowel
- Numbness around the saddle area (inner thighs, groin, buttocks)
- Rapidly worsening weakness in one or both legs
These can signal a rare but serious problem that needs immediate assessment at a hospital. If in doubt, treat it as urgent.
The Evidence
It very rarely needs surgery.
On its own, degenerative disc disease seldom calls for an operation. The sensible path is conservative care that keeps you moving, eases symptoms, and builds the strength and resilience that let your spine handle everyday load comfortably.
As with other wear-related findings, the honest aim of care is better function and fewer flares, rather than reversing the change itself. If something more serious is going on behind your symptoms, an assessment is how we identify it, and we will refer you if that is the right step.
