Stenosis has a tell: pain or heaviness in the legs that builds when you stand and walk, and eases when you sit down. Here is what it is, why that pattern happens, and how we approach it at Shift.
What It Is
A narrowing of the space your nerves travel through.
Spinal stenosis is a narrowing of the spaces inside your spine, where the spinal cord and nerves run. As that space tightens, the nerves can become crowded, and that is what produces the symptoms. It usually develops gradually, often as part of the wear that comes with age, and it most commonly affects the lower back, though it can also occur in the neck.
Like a lot of spinal findings, stenosis can show up on a scan without causing much trouble at all. So what your symptoms are actually doing, and how much they affect your day, matters just as much as the image. That is what a proper assessment is for.
The Giveaway
Why standing and walking are the clue.
Stenosis has a fairly recognisable pattern. Because bending forward tends to open the spinal canal a little, symptoms often ease in postures that round the back, and build in ones that arch it. These are the signs that commonly point to it.
Worse with standing and walking
Leg pain, heaviness, numbness or a tired feeling that builds the longer you stay upright or the further you walk.
Eases when you sit
Symptoms tend to settle soon after you sit down or lean forward, because those positions give the crowded nerves a little more room.
Easier leaning on a trolley
Many people notice they can walk further leaning on a shopping trolley, or going uphill, both of which bend the spine gently forward.
Knowing The Difference
When to seek urgent care.
Most stenosis is not an emergency and changes slowly. A small number of signs mean you should seek urgent medical care straight away rather than wait for an appointment.
Seek emergency care now if you have
- Loss of control of your bladder or bowel
- Numbness around the saddle area (inner thighs, groin, buttocks)
- Rapidly worsening weakness in one or both legs
These can signal a rare but serious problem that needs immediate assessment at a hospital. New problems with balance, coordination, or your hands and feet also warrant prompt medical review, even if they are not an emergency.
The Evidence
Surgery is not the automatic answer.
For many people, spinal stenosis is managed well without surgery. Guidelines generally support trying conservative care first, with a focus on staying active, improving mobility, and building the strength that supports your spine. Surgery tends to be considered for more severe or progressing cases, or where conservative care has not helped enough.
Stenosis is a structural change, so the honest aim of conservative care is to improve how you move and function and to ease your symptoms, rather than to promise the narrowing itself will reverse. Whether surgery is right for you depends on your specific situation, and if it is, we will say so and refer.

Not quite. Stenosis is a narrowing of the spaces inside the spine, often from gradual wear, while a disc problem involves the disc itself. They can overlap, since a bulging or herniated disc can contribute to narrowing, which is one reason a proper assessment matters.
A scan is not required to begin, but imaging is helpful for suspected stenosis because it can confirm the narrowing and show where it is. If you do not have one, we can guide you on how to get one.
